Die unerlaubte Weitergabe kontrollierter Medikamente ist ein wichtiges Problem, weil sie Suchterkrankungen fördern kann. In dieser Studie wollten wir herausfinden, wie Forschende Daten aus vielen Ländern und verschiedenen Sprachen gemeinsam auswerten können. Dafür analysierten Forschende in mehreren Ländern Medienberichte in ihren jeweiligen Sprachen und führten die Ergebnisse anschließend zusammen. Wir fanden heraus, dass diese Methode gut funktioniert und weltweite Vergleiche ermöglicht, ohne wichtige lokale Besonderheiten zu verlieren. Diese Ergebnisse können helfen, internationale Forschung mit mehrsprachigen Daten besser zu planen und umzusetzen.

Abstract

Hintergrund und Fragestellung
The diversion of controlled medicines, like narcotics and psychotropics, is a global concern as it contributes to non-medical use, misuse, and substance use disorders. However, understanding this problem globally is methodologically challenging as it requires knowledge of multiple languages and cultural contexts. This study addresses both these dimensions by examining diversion pathways across WHO regions while focusing on the analytic approach as a methodological contribution to addiction research.

Methoden
We conducted a qualitative content analysis of media reports, identified through structured, multilingual search strategies. A deductively developed coding framework was applied in the first analytic phase by researchers with language proficiency and contextual expertise in the respective countries. This enabled culturally sensitive coding within a standardized structure. Subsequently, relevant coded content was extracted and translated into English, creating a unified dataset. This dataset was then analyzed inductively to identify cross-regional patterns and themes.

Ergebnisse
Our approach enabled the inclusion of 492 media reports from 25 countries across all WHO regions. Methodologically, the approach ensured contextual fidelity in the initial coding phase and comparability through the shared coding framework, demonstrating multilingual coding feasibility. The subsequent translation and inductive analysis allowed for the identification of global patterns of diversion mechanisms and multi-actor involvement.

Diskussion und Schlussfolgerung
We demonstrated that combining deductive coding in original languages with inductive analysis in a common language is a robust strategy for global qualitative research in the field of addiction. This enables context-sensitive interpretation while ensuring comparability across settings. By systematically mapping diversion pathways as a driver of non-medical use and substance use disorders, the findings can inform more responsive surveillance, prevention, and policy strategies. Importantly, the methodological process itself represents a key contribution, offering a transferable model for addressing linguistic and cultural diversity without compromising analytical rigor or relevance for addiction research.
